Medical Spas

The Telegraph has reported the UK Plastic Surgeons’ concerns regarding the lack of regulation with Medical Spas.

These establishments appear to provide invasive treatments such as Botox and Fillers without any medical supervision! The Medical Spas seem to feel this is safe as long as they keep to a strict sense of hygiene.

I’m not sure I would agree with that. Anything that involves injections should only be done under medical supervision. Certainly the treatments done are safe. But only under the right hands. And hygiene standards alone do not guarantee the ability to deal with any problems that ensue. And complications do occasionally happen.

For example: granuloma formation with hyaluronic acid fillers. Would Aestheticians know how to recognise and treat such an occurrence? Ptosis with Botox: they can’t prescribe the required eye drops to enhance muscle activity.

Where will they stop? Syringe liposuction? Thread lifts?

Aesthetic procedures have been de-regulated in the UK recently. I was wondering what the controversy was about. Surely doctors, particular the ones trained in the UK, know their limits and would not overstep the boundary. Now I see the point of contention.

Once again, I back the Plastic Surgeons and say that procedures, even minimally invasive ones, should be performed by a medically qualified person only.


Melanoma Myths & Risks

Malignant Melanoma

Malignant melanomas account for more than 75% of deaths from skin cancer. Some cases are missed because of common myths regarding the disease.

1) Non-Caucasians can develop malignant melanoma with excessive sun exposure
2) Atypical melanomas can be mis-diagnosed as benign (not all melanomas have variation in colour, asymmetrical borders or size greater than 6mm)
3) Some hair-bearing moles can be cancerous

Caucasians are 10 times more likely to develop melanoma than other races.

African-Americans often develop melanoma in non-sun-exposed parts of the body: nails, soles of feet, palms of hand, mouth, nasal passages and genitals!

Only 33% of African Americans develop melanoma in sun-exposed areas!

The development of melanoma in non-exposed areas of the body results in spread of the cancer before diagnosis, and a high fatality rate. One American dies of skin cancer every 65 minutes. Appropriate lifestyle behaviour and vigilance plays a large part in the prevention and early detection of this disease.

Sun Block May Be Inadequate Protection Against Melanoma

bikini.jpgWearing sunprotective clothes and a hat and reducing sun exposure to a minimum should be preferred to sunscreens.

Often this solution is deemed to be unacceptable and sunscreens are usually the predominant mode of sun protection.
Sunscreens had been shown to protect against sunburn and less serious forms of skin cancer, said the experts. However, there was no conclusive proof that they prevented the potentially deadly melanoma skin cancer.

Nina Goad, of the British Association of Dermatologists, said: “Sunscreen is an effective tool, but it should not be used as the single method of sun protection.

Researchers have revealed that adequate protective clothing is more reliable at protecting against melanoma than sunblock alone.

Many people are lulled into a false sense of security and abuse low SPF sun protection during periods of sun exposure. The sensible use of sun block includes:

  1. Choosing the right SPF level for the sun exposure level. SPF 15-30 is needed for normal indoor use.
  2. Application 15-30 minutes before sun exposure.
  3. Liberal even application over all sun exposed areas.
  4. UV-A and UV-B protection. UV-B rays cause burning, but it is UV-A rays that cause skin cancers and premature ageing.
  5. Reapply if exposure is prolonged and if you sweat or swim.
  6. Choose a product that you can apply with ease. There are some that are non-greasy, absorb easily and smell wonderful!

Physical protection used in conjunction with a good sun block is ideal. Once again, sensible barrier choices are essential. I had a client who wore hats everywhere and was convinced she was protecting herself against the sun. However, the hats she preferred either had tiny rims so most of her face was not covered, or had large holes in the rim. Very pretty, but useless as far as sun protection is concerned.

Clothes can offer protection too. Again, clothes made of tightly weaved material is a more sensible choice.

Ultimately, common sense is essential. Some exposure with adequate protection and avoidance of prolonged exposure, particularly at the hottest times of day are vital.

Latest Acne Scar Treatment with Microneedle Therapy

The aim of acne treatment is not just to reduce outbreaks, but the prevention of permanent scars. For adults whose acne has been quiescent for years, the scars often remain. Such permanent facial scarring, especially if prominent and deep over the cheek area, can have a major negative impact on the individual’s self confidence.

Various treatments have been used in an attempt to reduce the appearance of acne scars. Microdermabrasion is popular as it is relatively non-invasive. Multiple sessions (20-25) are needed for optimum results. The scars depth is reduced and a glow is more visible. Microdermabrasion is useful in shallow, rolling scars and has little impact on the deeper ice pick scars.

Subcision has been employed to cut the fibers under the scar tissue, in an attempted to release the scars from the underlying tissues. This is useful for ice pick scars. Further filling with a temporary or permanent dermal filler may be needed for an even skin surface.

The best treatment to date has been laser resurfacing. This destroys the skin surface. When the epithelium grows back, the skin is smooth and unscarred. However, this is a traumatic procedure with a prolonged recovery.

Say Good bye to Tired Eyes & Eye Bags

It is often said that our eyes are the windows to our souls. A quick glance can reveal our current states of emotion, with happiness, fatigue and exhaustion being most visible. The delicate eye area is also vulnerable to early signs of aging.

Many people express frustration at looking chronically tired despite getting adequate rest. Eyebags, fine lines and hollowing under the eyes (tear trough) are common signs of ageing. So if getting the prescribed eight hours of sleep every night and religiously applying the latest eye creams don’t solve these problems, is it time to resort to surgery?

What is perceived as eyebags can be from a variety of causes:

When seen in a young person, particularly someone that squints from myopia, this could be due to over-activity of the eye muscles (orbicularis hypertrophy). Such eye bags are usually well defined and more obvious when the person squints. The eye bag is also closest to the eyelid. Such eye bags respond well to Botox, which has an added effect of widening the eye aperture, resulting in larger eyes!

Eye bags can also be caused by excess fat tissue in the area under the eyes. There are 3 distinct fat pads, lying in a straight row below the eyes. Simply A-Peeling - All About Chemical Peels

Recent advances in technology have seen the introduction of numerous machines all promising the reversal of the signs of ageing. From Intense Pulsed Light (IPL), to Laser, to Radiofrequency to Light Emitting Diode (LED) Technology, the various machines all claim to stimulate collagen production and tighten up sagging skin. The results are often subtle and may take months and multiple sessions to appear, correlating with the time it takes for new collagen to form.

Oft neglected in aesthetic marketing, possibly from its sheer simplicity, is the humble chemical peel. This category of treatment has also undergone drastic advances in the recent years, and the results seen often parallel those seen with more expensive machine treatments!

Chemicals have been used since the time of Cleopatra, whose milk baths are seen as early lactic acid peels. Chemical peels have long been used by dermatologists in the treatment of acne and pigmentation, as well as reverse the signs of ageing.

As skin ages, the rate of cell turnover slows down, resulting in the buildup of a thick layer of dead squamous cells. This causes the dull, lackluster appearance commonly seen as people age. Other signs of ageing include wrinkling, pigmentation (sun damage), loss of volume in the cheeks and lips and sagging in the jowl area.

Effective anti-ageing aesthetic therapy should reverse all the above signs of ageing. The selection of the correct chemical peel, plus the use of appropriate skincare on a daily basis, can produce such results in a matter of weeks!

However, the choice of peel is very important, as is the choice of operator. Chemical peels can have the same name, but if done at a beauty salon, has lower concentrations of acid than a seemingly identical product used by a doctor.

Chemical peels can be broadly categorized into alpha-hydroxyacids (AHA), beta-hydroyacids (BHA), salicylic acid, tricholoracetic acid (TCA) and phenol.

AHAs and BHAs include Glycolic Acid and Lactic Acid. These are relatively simple peels which remove the excess dead skin cells and stimulate collagen production.

Peels containing salicylic acid are useful in the treatment of acne. Salicylic acid helps to reduce the plugging of the skin pores, hence reduces the formation of comedones.

TCA technology has advanced dramatically over the recent years. The addition of modifying agents to the solution has made this form of chemical peel safe, versatile and eminently controllable. Take the Easy TCA Peel from Skin Tech (Spain) as an example. Clear skin colour changes indicate the depth of peel. Hence this one peel can be used for simple rejuvenation (light peel required), to the treatment of melasma (large areas of deep pigmentation), to the elimination of pigmented lesions (sun damage) and fine lines. The Easy TCA peel requires four treatments, spaced a week apart. The entire treatment is completed after a month with marked improvements visible between baseline and follow-up photographs.

Phenol peels are the strongest form of chemical peels. Pure phenol peels are not available locally. Some peels have a small concentration of phenol included. These peels tend to have dramatic lifting effects.

Chemical Peels are generally very quick procedures, taking on average 5 minutes to complete each session. Depending on the type of peel chosen, gentle flaking is often seen for 2-3 days. This can easily be camouflaged with moisturizers. Some downtime can be expected if deep peeling is performed to reduce difficult pigmentation.

Chemical Peels do increase skin sensitivity to sunlight. Optimal sun protection is essential during and after the peeling process. Sun exposure without inadequate protection can result in a worsening of existing pigmentation. However, this is the situation with most aesthetic treatments, including PL and laser treatment.

Some are concerned about permanent skin thinning after chemical peels. Although the excess dead skin cells are sloughed off in the process, this actually triggers the skin to produce more collagen, hence increasing skin thickness and produces a lifting effect.

Chemical peels can be repeated on a regular basis (more frequent treatments may be needed initially) or at least once a year. They are synergistic with the other treatment modalities available, such as mesoglow (infusion of essential vitamins directly into the skin) and Botox. Their safety, efficacy and flexibility make this group of treatments simply a-peeling!

We liken it to orange peel or cottage cheese. Women have it, so do men. Cellulite, also known as dermatomyoliposclerosis, affects the young, the old, the fat, the slim, the gym-sculpted and the couch potato. General estimates say up to 90 per cent of women have it. But can we do anything about cellulite? Cellulite is a disease of the fat level of skin. The fat cells are arranged in chambers surrounded by fibrous bands. When there is a build-up of toxins and poor lymphatic drainage, water retention occurs and cellulite forms. The fibrous bands can contract, resulting in a permanent dimpling of the skin.

Unfortunately, once formed cellulite is hard to dispel with lifestyle changes alone. Many have suggested drinking more water, exercising and eating healthily, but none of these are effective. Even liposuction, often touted as the cure for cellulite, can worsen the situation. Hormonal changes also contribute to cellulite, which partly explains why women are more prone to it, and genes are thought to play a part. Wrinkles, deep folds and fine lines are signs that we are no longer are as young as we feel. These can be dynamic (appear with facial expression) or static (there all the time). Dynamic wrinkles are caused by muscle activity and become static and deeper with time.

Many of us yearn to look youthful without plastic surgery or having a face frozen by Botox injections. Fortunately, this is possible and easily achieved with a judicious mix of Botox and injectable fillers.
